Community Based Care Management (CBCM)
Community Based Care Management (CBCM) assists Fayette County Residents with securing access to their Identified Social Determinants of Health (SDOH) including Secure / Safe Housing, Employment. Transportation, Clothing, Food, Child Care, Utility Assistance, Physical Health, or Financial Strain.
